C22.214.171.124. Originally Classified Documents. Every originally
classified document must have a "Classified by" line placed on the face of
the document that identifies the original classification authority
responsible for classification of the information it contains. The original
classification authority shall be identified by name or personal identifier
and position title. If the information normally included on the "Classified
by" line would reveal classified information not evident from the rest of
the document, the "Classified by" line should be completed with an
unclassified personal identifier that can be traced through secure channels.

document, identify the guide or document on the "Derived from" line.
